Financing Information
The practice accepts Visa, MasterCard, Discover Card, and American Express.
In addition to these sources, we offer patients financing options through
the Chase Bank loan by phone program. Chase provides rapid approval processes together with reasonable
repayment provisions to patients with approved credit. Surgery schedulers
will be happy to assist you with this in conjunction
with your visit to our office.
Insurance Requirements
Breast
Reductions:
Coventry, Cigna,
Fiserv Health, Benefit Management, Humana, Blue Cross (out of state plans)
and Gilsbar:
1.
Documentation that the member has a history of two or more visits in one
year to a primary care doctor with complaints of two or more of the
following symptoms, headaches, upper back pain, neck pain and/or shoulder
pain.
2.
Documentation of treatments that were tried and failed. Such as any
types of anti-inflammatory or other medications taken for the above
mentioned symptoms.
3.
Documentation that physical therapy or chiropractic care has been
unresponsive to a 3 - 6 month course of treatment.
Aetna:
1. In
addition to the above a patient that is 40 years of age or older must also
supply a copy of recent mammogram.
Please note that all of these requirements must be met for us
to receive approval from your insurance company and the documentation should
be brought with you for your initial consultation.
